Your healing journey begins with a call.
The admissions process at Clearview Outpatient starts with a simple phone call. During this call, our compassionate and knowledgeable team will collect essential clinical and demographic information to begin the assessment process.
After thoroughly reviewing the information provided, our admissions team will determine the suitability of our programs for the candidate. If we’re unable to offer admission, we’ll clearly explain the decision and provide recommendations for alternative treatment options, ensuring every individual finds the care they need.
If the prospective client is currently at another treatment facility or has recently been hospitalized, we may request specific records for clinical review, such as:
- Most recent psychiatric evaluation
- Recent history and physical exam
- Current psycho-social history
- Clinical notes (if currently in a treatment program)
- A list of current medications, including dosages and frequencies
- Other relevant clinical or educational records
Admissions Eligibility Requirements
